APNA Nurse-led Diabetes Clinics for Practice Nurses

Turn your ideas for improving diabetes nursing care into reality with APNA.

This year, Murrumbidgee Primary Health Network is continuing to focus on supporting general practices to care for patients with chronic disease in our region.
 
We are pleased to announce we are offering five nurses the opportunity to take part in APNA's Building Nurse Capacity Program, with the aim to improve the health and wellbeing of your patients living with Type 2 diabetes.
 
APNA's Building Nurse Capacity Program supports nurses in developing nurse-delivered, team-based models of care, also known as nurse clinics. So far, they've supported over 50 clinics around Australia. 
 
Over 18 months, nurses implementing nurse-led diabetes clinics will be supported by APNA's expert team to build their ideas from the ground up, ensuring they have considered all the essential elements for success.    
 
As well as all the practical support offered by APNA, participating practices will receive a $12,000 incentive payment, co-sponsored by Murrumbidgee PHN.
